Fall 2010 Registration
U5 through U15 Registration IMPORTANT CHANGES

Registration begins June 28th and ends August 6, 2010, to guarantee your place on your old team, you must register by this date, no exceptions, there is no late registration.

Wait list begins August 8th, if teams need additional players, players will pulled off this list in the order they signed in.

Refund policy has changed.

New rates U5 & U6 = $55 U7 & up = $65
Referee will not be provided for U5 & U6 games.
Proof of residency will be required for all 3rd child discounts, a copy of a report card will need to be submitted for each child to verify home address.

Where to Register |
: Please click on the appropriate registration link below. You will be taken to our registration page. Please follow the instructions on-line to register.
Proper payment –PAYMENT MUST BE RECEIVED OR OTHER ARRANGMENTS MADE PRIOR TO PLAYER BEING PLACED ON TEAM.
1. From home - register on-line and pay by credit card, online check or you can bring or mail your payment to the MSA office. Please note your registration is not complete until which time payment has been recevied.
2. MSA Office has 3 computers in the front of the office for registration. |

Special Requests |
1. New Under 5 and Under 6 players may request to play with a friend – the “Buddy Rule”.
2. No player, Under 7 and older, can request a specific “buddy” or team and will automatically be placed into the blind player pool. No exceptions!
3. Special requests will only be accepted from a parent and must be in writing and received by registration deadline. The request will be reviewed by the commissioner. Only the commissioner may approve special requests. |

Returning Players |
You should have received an email with your username and password that has brought you to this page. If you do not have your user name or password click on the link at the bottom of the page.
Click on the link below and fill out the form completely.
Birth certificate is not necessary for returning players unless you have been contacted by your coach or commissioner that MSA needs one. |

New Players |
New players only - A copy of your child’s official, government issued birth certificate (not hospital certificate) must be emailed, faxed or dropped off at the MSA office. All birth certificates must be in the MSA office by the deadline date in order for the player to be placed on a team. |

Returning Players to the Player Pool |
Any returning player that does not want to return to their team from the fall season may go in to the player pool for their age group. DO NOT register as a returning player. Register as a new player and request to be put into the player pool in the notes section at the bottom of the form.

Refund Policy |
|
There will be a $10.00 fee for all refunds before player is placed on a team. Once player is on a team the refund will be 50% of the total amount paid. No refund after season starts. |
